Your first hygiene patient is checking in. The doctor is reviewing the day. Two operatories are being turned over. The front desk is confirming insurance.
Then somebody says:
“The schedule won’t load.”
A second employee says the X-rays are missing.
The phones begin cutting in and out.
Someone restarts a workstation.
Someone else restarts the server.
The practice-management software vendor says it looks like a network issue. The internet provider says the connection is online. The imaging vendor says its software is working.
And suddenly, nobody is quite sure who owns the problem.
That is the reality of dental technology: a practice does not run on one computer system. It runs on an interconnected chain of systems.
Internet. Firewall. Switches. Wi-Fi. Servers. Workstations. Practice-management software. Imaging. CBCT. Phones. Microsoft 365. Backups. Remote access. Vendor integrations. User accounts.
One weak link can affect the entire day.
That is why Legend Networking developed the Dental IT Failure Map: a practical, room-by-room way to identify technology risks before they become downtime.
This is not a HIPAA compliance certification, a substitute for a formal security risk analysis, or a legal checklist. It is an operational framework designed to help dentists, office managers and dental groups ask a much better question:
If one part of our technology failed tomorrow morning, would we know exactly what broke, who owns it and how we would keep treating patients?

What Is a Dental IT Failure Map?
A dental IT failure map identifies the systems that your practice depends on to operate and then asks three questions about each one:
- What happens if this system fails?
- What other systems depend on it?
- How quickly can the practice recover?
The goal is not to eliminate every possible technology problem.
That is impossible.
The goal is to eliminate unknown single points of failure.
A well-designed dental technology environment should make it easier to answer:
- Where patient data lives
- Which systems are business-critical
- Which vendors have remote access
- Which devices depend on the internet
- Which applications depend on the local server
- Where imaging files are stored
- Whether backups can actually be restored
- What happens when the primary internet connection fails
- Who can access Microsoft 365 and cloud applications
- What the practice will do during a cybersecurity incident
- How the team operates if normal systems are temporarily unavailable
Below are 12 places we would examine first.
1. Your Internet Connection Is a Single Point of Failure
For many dental practices, the internet now supports far more than web browsing.
It may support:
- Cloud-based practice software
- Insurance portals
- VoIP phones
- Credit-card processing
- Cloud imaging
- Microsoft 365
- Patient forms
- Online scheduling
- Remote support
- Cloud backups
- Vendor portals
If one internet circuit goes down and there is no tested backup connection, multiple unrelated systems may appear to fail simultaneously.
That can create confusion because the phones, software and payment systems may all look broken even though the original problem is simply connectivity.
What to check
Ask:
- Who is our primary internet provider?
- What is our current circuit speed?
- Do we have a secondary connection?
- Is failover automatic or manual?
- When was failover last tested?
- Will our phones continue working during an ISP outage?
- Will cloud software remain accessible?
- Does our firewall support dual-WAN or cellular failover?
A backup connection that has never been tested is not the same thing as a documented business-continuity strategy.
2. The Firewall and Network Core Are Running the Entire Practice
The firewall is often the gateway between the dental office and the outside world.
The switches distribute connectivity.
The wireless access points connect mobile and wireless systems.
When this layer is poorly configured, outdated or undocumented, the symptoms can appear everywhere else.
You may experience:
- Random software disconnects
- Slow image loading
- Intermittent Wi-Fi
- Dropped phones
- Failed cloud backups
- Inconsistent remote access
- Devices that cannot see the server
- Imaging workstations that disappear from the network
The dangerous part is that the problem may not look like a network problem.
What to check
Confirm that your IT team can answer:
- What firewall model are we using?
- Is its firmware supported and maintained?
- Who has administrative access?
- Are network configurations backed up?
- Are clinical devices separated appropriately from guest traffic?
- Are switches managed and documented?
- Are there unknown consumer routers or wireless extenders plugged into the network?
- Is there a current network diagram?
A network should not depend on somebody remembering how it was built three years ago.
It should be documented.
3. Your Wi-Fi May Be Supporting More Clinical Work Than You Realize
Modern practices increasingly depend on wireless systems.
Tablets, laptops, scanners, patient forms, mobile imaging devices, printers and other connected equipment can all depend on Wi-Fi.
Poor wireless design can create dead zones, roaming issues and intermittent connectivity that staff members interpret as “the software being slow.”
What to check
Walk the entire practice with the systems your team actually uses.
Pay particular attention to:
- Operatories
- Consultation rooms
- Imaging areas
- Check-in/check-out
- Sterilization
- Doctor offices
- Hallways between access points
Do not evaluate Wi-Fi only from the front desk.
A signal that looks strong at reception can perform very differently behind lead-lined walls, cabinetry, equipment and dense construction materials.
4. The Server Can Be Healthy — Until One Component Is Not
Many dental environments still depend on an onsite server or a hybrid local/cloud architecture.
That server may host:
- Practice-management databases
- Imaging data
- Shared files
- Domain services
- Applications
- Licensing services
- Backup jobs
- Vendor databases
A server problem does not always begin with a dramatic failure.
Warning signs may include:
- Low disk space
- Failing storage
- Backup errors
- Unexpected reboots
- Old operating systems
- Unsupported hardware
- Overloaded virtual machines
- Unresolved hardware alerts
- Improper shutdowns
- No functioning UPS
What to check
You should know:
- Server age
- Warranty status
- Operating-system support status
- Available storage
- RAID/storage health
- UPS health
- Backup status
- Virtual-machine configuration
- Replacement plan
- Recovery procedure
The worst time to learn the age and configuration of your server is while the practice is already down.
5. A Successful Backup Is Not the Same as a Successful Recovery
This may be one of the most important distinctions in dental IT.
A dashboard saying “Backup Successful” tells you that a backup job ran.
It does not automatically prove that your practice can recover.
A resilient backup strategy should account for both operational failure and cybersecurity events.
That may include a combination of:
- Local recovery
- Offsite backup
- Protected or isolated copies
- Defined retention
- Encryption
- Monitoring
- Documented recovery procedures
- Routine restore testing
The question that matters
Do not ask only:
“Are we backed up?”
Ask:
“When was the last time someone restored our data and verified that it worked?”
Then ask:
- What exactly is backed up?
- Where are copies stored?
- How many recovery points exist?
- Can the backup environment be reached from normal administrator accounts?
- Who receives failed-backup alerts?
- How quickly could the practice access critical systems after a server failure?
- When was the last documented recovery test?
The objective is not simply to possess a backup.
The objective is recoverability.
6. Practice-Management Software Depends on More Than the Software Vendor
Whether your practice uses Dentrix, Open Dental, Eaglesoft, Curve, Archy or another platform, the application lives inside a larger technology environment.
Performance can be affected by:
- Workstation health
- Server performance
- DNS
- Network latency
- Permissions
- Database connectivity
- Windows updates
- Third-party integrations
- Security tools
- Imaging bridges
- Internet connectivity
This is why finger-pointing can happen during an outage.
The software vendor may see a network issue.
The ISP may see an active internet circuit.
The imaging vendor may see its application running.
The IT provider has to understand how those systems connect.
What to check
Your IT documentation should identify:
- Where the database lives
- Which workstations run critical software
- Required network paths
- Vendor support information
- Licensing dependencies
- Imaging bridges
- Backup requirements
- Integration points
- Remote-access methods used by the vendor
Dental IT is not just computer repair.
It is vendor coordination across a clinical technology ecosystem.
7. Imaging and CBCT Can Become Their Own Technology Island
Imaging is one of the most specialized parts of the dental technology environment.
A practice may rely on:
- Intraoral sensors
- Panoramic systems
- CBCT
- Imaging workstations
- Acquisition PCs
- Viewing software
- DICOM
- TWAIN drivers
- Bridges
- Shared image databases
- Vendor-specific services
If the practice-management system works but imaging does not, the office can still experience serious disruption.
What to check
Document:
- Where images are stored
- How images are backed up
- Which machine controls acquisition
- What software version is installed
- Which bridges connect imaging to the PMS
- Which devices require static IP addresses
- Which vendor supports each component
- Whether replacement workstations have the required drivers and configuration
- Whether imaging can be recovered independently from the PMS
Do not assume that because patient charts are backed up, imaging is automatically protected the same way.
Verify it.
8. Every Workstation Is Both a Clinical Tool and a Security Endpoint
The computer in an operatory is not “just a PC.”
It may have access to:
- Patient records
- X-rays
- Insurance systems
- Payment applications
- Practice-management software
- Shared drives
- Cloud applications
That means workstation health matters operationally and from a cybersecurity standpoint.
What to check
Look for:
- Unsupported operating systems
- Missing security updates
- Local administrator access
- Unapproved software
- Unapproved remote-access tools
- Disabled security software
- Shared user accounts
- Stored passwords
- Old devices that have been forgotten but remain connected
9. Microsoft 365 and User Identity Are Part of Dental IT Now
The server room is no longer the only place a dental practice stores valuable information.
Email and cloud identities can provide access to:
- Messages
- Attachments
- Shared documents
- Calendars
- Vendor communications
- Financial information
- Password-reset emails
- Cloud applications
That makes identity protection a critical part of the technology environment.
What to check
For Microsoft 365 and other cloud systems, verify:
- Multi-factor authentication
- Separate accounts for each employee
- Removal of former employees
- Administrator-role restrictions
- Suspicious-login monitoring
- Inbox forwarding rules
- Recovery-account information
- Secure password-reset procedures
One shared login used by half the office may feel convenient.
It also makes accountability, access control and offboarding much harder.
10. Remote Access Can Be Helpful — and Still Needs Tight Control
Dental practices legitimately need remote support.
Software vendors, imaging vendors and IT providers may all need access.
The issue is not whether remote access exists.
The issue is whether the practice knows who can access what, with which tool, under whose account and for how long.
What to check
Create an inventory of:
- Remote monitoring and management tools
- Remote desktop software
- Vendor support applications
- VPN accounts
- Administrative accounts
- Permanent vendor access
- Temporary support access
Then ask:
- Is each tool approved?
- Is MFA used where supported?
- Are old vendor accounts disabled?
- Are access logs available?
- Does every technician use an individual identity?
- Can the practice identify an unexpected remote session?
Remote support should be controlled, intentional and documented.
11. Your Phones May Depend on the Same Systems You Are Trying to Troubleshoot
Many practices use VoIP phones.
That creates advantages, but it also creates dependencies.
Phones may rely on:
- Internet
- Firewall configuration
- Switches
- Power over Ethernet
- DNS
- Cloud phone services
When internet or network equipment fails, phones may fail at exactly the moment the practice needs to communicate with patients.
What to check
Know:
- Whether phones are cloud-based
- Whether they survive primary ISP failure
- Whether network equipment is protected by UPS
- Whether calls can forward to mobile devices
- Who can modify the phone system
- What the emergency communication plan is
A practice continuity plan should account for communication, not only data.
12. The Biggest Failure May Be Not Having a Plan
A dental office can have good technology and still be poorly prepared for an incident.
From an operational standpoint, that means the practice should know what it will do when technology does not behave normally.
Your practice should have answers for:
- Who declares a technology emergency?
- Who contacts IT?
- Who contacts software and imaging vendors?
- Who communicates with employees?
- How does the front desk operate temporarily?
- How are patients contacted if phones are unavailable?
- Where is the vendor contact list stored?
- What happens if email is compromised?
- What happens if the server is unavailable?
- What happens if the building loses power?
- What happens if ransomware is suspected?
- What systems must be restored first?
The important word is preparedness.
